GENERAL SUMMARY:

Responsible for meeting patient priority needs by assessing, participating in interdisciplinary care planning, providing and /or delegating care and evaluating patient responses and progress toward clinical outcomes. Contributes to an efficient and effective work environment through activities including professional communication and documentation, precepting and/or resourcing of others.

Supervises Licensed Practical Nurses and/or other support staff in the delivery of patient care as described in Nebraska Statute 38.

PRINCIPAL JOB FUNCTIONS:

  • Commits to the mission, vision, beliefs and consistently demonstrates our core values.
  • Initiates assessment and reassessment based on the patient’s presenting problems/needs in accordance with Bryan Medical Center’s procedures.
  • Synthesizes pertinent initial and ongoing patient data to identify nursing priorities for care and desired outcomes/goals.
  • Integrates appropriate standards of care into the patient’s plan of care based on the diagnosis and patient-specific needs.
  • Executes therapeutic and/or supportive interventions.
  • Provides education for the patient and family to meet identified patient needs according to their needs, abilities, preferences and readiness to learn.
  • Evaluates the patient’s response to and effectiveness of interventions.
  • Evaluates patient’s progress toward established outcomes.
  • Incorporates cultural considerations into the plan/delivery of care.
  • Includes family, significant other, or identified support person in the care of the patient as appropriate.
  • Documents patient care and related information according to Bryan Medical Center procedures for documentation.
  • Provides care in collaboration with other members of the interdisciplinary team.
  • Facilitates patient’s transition to the next level of care.
  • Assumes responsibility in a shared or delegated assignment and supports team member’s practices through timely communication.
  • Serves as a role model to colleagues, students and other care providers.
  • Actively participates in continued learning opportunities and continued self-development pertinent to the work unit and duties of the job.
  • Integrates evidence based research findings into practice in accordance with policy and procedure.
  • Engages in department specific and organizational performance improvement initiatives.
  • Appropriately utilizes all safety equipment when indicated and as defined by organizational procedure (i.e. lifting equipment, PPE, sharps, etc.).
  • Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
  • Participates in meetings, committees and department projects as assigned.
  • Performs other related projects and duties as assigned.

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • Maintains clinical competency as required for the unit of assignment including but not limited to population-specific competencies and participates in annual skill competency verification.
  • Knowledge of federal and state regulations related to nursing applications and relative practice area.
  • Knowledge of computer hardware equipment and software applications relevant to work functions.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to delegate, provide instruction and evaluate the work of LPN’s and unlicensed professional
  • Ability to maintain confidentiality of patient information.
  • Ability to exercise professional judgment, courtesy and tact in daily interactions with others.
  • Ability to establish and maintain effective working relationships with health care team members and diverse patient populations.
  • Ability to work in a fast-paced environment relevant to changing patient needs, including work with patients with acute, chronic, and complex disease processes, and those who are dying.
  • Ability to maintain regular and punctual attendance.

EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations.

OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.

ADDITIONAL JOB FUNCTIONS:

In addition to the principal job functions contained on the primary job descriptions, the following duties are also required in the department noted above.

  • The Deterioration Detection Registered Nurse is a member of the collaborative team who is responsible for providing continuous vigilance to a diverse population of adults across inpatient units.
  • Provides proactive, continuous monitoring, prevention, early intervention and ongoing care with the goal to decrease morbidity and mortality.
  • Utilizes evidence based care and clinical decision support to guide practice.
  • Acts as a patient advocate and resource to inpatient nursing staff.
  • Collaborates with the High Risk Coordinator and DDC Lead RN.
  • Reports to ICU manager from primary unit of employment.
  • Analyzes and interprets visual and summarized data findings utilizing databases and statistical programs in a proficient and timely manner.
  • Provides consultation regarding patient assessments, data abstraction, quality outcomes and compliance, and promotes an understanding of evidence-based clinical practices for medical, clinical and ancillary staff.
  • Participates in mortality reviews and case reviews with healthcare team as needed.
  • Prevents clinical problems from occurring by proactively surveying adverse physiological trends, assuring ordered prophylactic strategies are appropriate, reviewing medical charts, and responding with appropriate actions to alerts

ADDITIONAL REQUIRED CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network. Advanced Cardiac Life Support (ACLS) certification required.
