QUALIFICATIONS:
- LMSW, LCSW or LCSW-R with a NYS License to practice, LMHC, or Licensed Psychologist
JOB SUMMARY: The Behavioral Health Therapist provides counseling services, ensuring wholistic, high quality care to patients.
Essential Job Duties
PATIENT CARE
- Maintain a caseload of approximately 30-35 one-hour patient sessions per week, dependent on program needs
- Engage patient in a healthy therapeutic relationship
- Advocate for patient needs when necessary
- Refer patient to needed care within JRCHC or to outside resources
- Provide individual, group, or family therapy
- Route patients to appropriate community programs and teams
COLLABORATION
- Collaborate with Providers, Interpreters, and Care Coordinators to ensure holistic care
- Take initiative with patients and Behavioral Health Team needs
- To facilitate and triage referrals from various referral sources
- Coordinate skilled services for comprehensive case management
- Demonstrate positive interrelationships and service excellence in the performance of duties by meeting or exceeding expectations
ADMINISTRATION
- Support the integration of Behavioral Health within a Medical Model
- Complete tasks assigned by BH Director on an as needed basis
- Maintain confidentiality of records relating to clients' treatment
- Adhere to the standards outlined in the Code of Ethics
- Complete necessary paperwork in a timely manner
- Submits service activity logs, weekly and monthly schedules, time sheets and mileage to the Behavioral Health Manager.
- Ensure licensure is in good standing, including upkeep of continuing education requirements
- Develop treatment plans for patients
- Complete thorough patient assessment

Individuals who hold positions at Jericho Road Community Health Center should possess the following qualification and capabilities:
- LMHC and LCSW are eligible to apply for the NHSC Loan Repayment Program, dependent on JRCHC’s HPSA score at the time of application
- Therapist has potential to be EMDR trained upon completing six months of employment
- Once a therapist is EMDR trained they will have access to a CEU budget under the discretion of the BH Clinical Manager.

- Experience working with EMR, Medent preferred
- Project-management experience
- Strong organizational skills and attention to detail
- Proficiency in Microsoft Office programs and Google suite of programs or willingness to learn

- Strong self-motivation
- Ability to travel between sites if necessary to serve multiple locations
- Experience working with low-income, diverse, multilingual populations highly desired

- Attend required training as deemed mandatory by organization or to continue education in field
PHYSICAL REQUIREMENTS: Must be able to sit or stand for long periods of time. Exert up to 50lbs. pushing, pulling, or lifting. Walking, climbing stairs required. Able to talk/hear and communicate with others with or without reasonable accommodation.
TRAVEL REQUIREMENTS: Must be able to travel to sites while maintaining standards of safety and compliance with motor vehicle traffic laws. This position may require travel for necessary training and conferences.
