We are seeking qualified candidates for a part-time Transition Coach. The Transition Coach will provide services and support to students in community settings related to obtaining and maintaining employment.

Job Duties Include:

• Coach students with various exceptionalities to improve employment outcomes after graduation from high school
• Read student’s Individualized Education Plans (IEPs) to understand how best to accommodate their learning needs while coaching at local job sites
• Build relationships with students to understand how they learn
• Implement behavior strategies to assist students as they learn different jobs
• Learn the various tasks students perform at job training sites and creatively find additional job tasks to support optimal learning experiences for students
• Complete daily computerized reports that reflect student progress and attendance
• Assist students with career acquisition skills such as communication
• Assist students with developing self-advocacy skills
• Works to recognize when students require more support or less support at job training sites
• Communicates with employers regarding student duties and successes
• Prepares and submits Transition Coach IEP input reports and participates in IEP meetings
• Performs other duties as assigned by the Executive Director or designee

The pay rate for this position is $17.00 per hour. The schedule for this position is up to 29 hours per week and the location is Greensburg, PA and surrounding areas within Westmoreland County.

Minimum Education: HS Diploma or Equivalent Required, AA/AS Preferred

Minimum Experience: 0-3 years of experience required

Additional requirements for this position include Act 34, 114 (federal criminal history) and 151 clearances, Act 126 training, School Personnel Health Record (physical), and Tb Test. EOE
