We are seeking a Service Coordinator to join our team!

Service Coordination is the process of working with participants under a waiver program in order to identify, coordinate, and facilitate all necessary health / medical services based on their needs and goals. You will provide administrative support to clients in either individual or group settings.

Responsibilities:

• Assess nature of clients' situations by interviewing and reviewing personal history
• Develop and execute individual treatment plans
• Determine appropriate milestones to gauge client progress
• Communicate client updates to all relevant parties
• Facilitate referrals to other healthcare professionals and programs
• Maintain accurate client documentation
• Meet important deadlines & requirements
• Travel throughout Pittsburgh & surrounding counties

**Must have reliable transportation- The SC conducts 4 face to face visits per week.

*** Must have a valid driver license.

Qualifications:

Bachelor’s degree in social work, psychology, human services or related field

OR

At least 3+ years of experience in social service or health care related setting

• Ability to prioritize & meet deadlines

*We are also looking for bilingual Coordinators ( Spanish, Russian, Chinese, Vietnamese, Korean, etc. )

Job Type: Full-time

Pay: $19.00 - $20.00 per hour
