The Triage Specialist Manager at La Colaborativa will play a leadership role in creating reliable and consistent access to programs, resources, and needed supports for the organization's 10,000+ Latinx immigrant members. The Manager will lead a team of community-based case managers dedicated to restoring stability and dignity to the lives of our members. The Triage team will be based from La Colaborativa's new Chelsea Survival Center and will manage client intakes, service provision models, and follow-up to ensure members are able to move out of survival mode, towards stability and empowerment as community leaders.

The Manager provides leadership and administrative oversight to a team of case managers to:

• Improve member access to community resources, including housing, food, public benefits, clothing, jobs, education and training, local schools, health care, behavioral health, legal aid, domestic violence support, etc.
• Coordinate and monitors services, including comprehensive tracking of members activities.
• Assist members in developing goals and areas of need.
• Document all client encounters and contracts made on behalf of members.
• Connect members to other programs at the organization to ensure a sustainable plan.
• Identify and provide emergency crisis services as necessary, assessing and responding to emergency cases according to accepted crisis intervention methods and techniques.

Education and Experience Requirements:

• Bilingual in Spanish and English required
• Bachelor degree in social work, or related field
• Higher degree and/or licensure in social work or related field preferred
• 1+ year experience in managing teams, implementing projects and working in public health field
• Cultural safety and sensitivity, specifically with Latinx and immigrant communities
• Enthusiasm for case management and community outreach
• Excellent verbal communication
• Excellent people skills
• Integrity
