Hours: 8:00 am-4:30 or 8:30 am – 5:00 pm, depending on the needs of the facility

Position Summary:
Wound Care Coordinator functions as expert in acute and rehabilitative management of patients with selected disorders of the gastrointestinal, genitourinary and integumentary systems. Provides consultative services to assist in the appropriate evidenced based and cost effective clinical management of patients with ostomies, fistulas, wounds (acute and chronic), pressure ulcers and incontinence related problems. The Wound Care Coordinator functions autonomously, independently and/or collectively, interfacing with every service unit, manager, ancillary service and clinicians in the organization as needed. Share clinical expertise to assist the clinical team in providing the treatment/care to the patients to achieve the best possible clinical outcomes.

Duties & Responsibilities:
Assesses the physical/psychological condition and identifies the needs of each patient through direct observation and examination. Collaborates with the healthcare team to ensure early identification and management of pressure ulcers, traumatic and draining wounds, surgical incisions, tubes and fistulas. documents assessments and treatments per facility policy. Assesses patient/family knowledge and ability to understand/or learn care techniques. Provides education to residents/patients and significant others regarding the care of wounds, ostomy, skin alterations, etc. Monitors and interprets assessment data and identifies appropriate plan of care and provides assessment findings to provider as needed. Collaborates with the providers, to initiate treatments, provide recommendations and follow up on progress of treatment. Communicates/documents assessments to inform the health care team. Provides assistance to staff in developing a plan of care and management for residents with draining wounds, stomas and/or skin breakdown. Plans Patient care to reflect goals that are realistic, measurable, and consistent with the medical plan of care for each patient (include discharge planning). Provides education to patient and significant other regarding the care of stoma and peristomal skin, appliance, and any special consideration/needs that should be reported to the physician. Assist with instruction in usage of an appropriate pouching system and accessories, including where and how to obtain supplies, when ready for discharge. Follows treatment of chronic wounds, fistulas or percutaneous tubes through assessment of patients, initiates appropriate referrals for treatments, management and/or medical surgical interventions. Educates staff to enhance the outcome of the residents. Provides consistent instruction and/or documentation of care. Accountable for in servicing new hires and providing ongoing education of existing licensed and non-licensed support staff. Assist staff to maintain current knowledge and competencies: orientation, schedule of in-service programs, online resources and review of policy and procedures according to national guidelines. Develops educational materials for staff and residents/staff. Follows and performs all related unit specific policies and procedures, as detailed on the unit specific competency check-list. Utilizes job and unit specific equipment as required. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Performs all other related job duties as assigned.




Qualifications


Experience:
Generally requires 3 to 5 years of related experience.

Education:
Bachelor's degree in Nursing is required.

License Certification:
Valid Florida RN license is required. American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Providers certification with at least 6 months validity required. Must meet and maintain current all unit specific and organizational skills/competencies, certifications/licensures, as required by the nursing area. Certification in Wound Care/ Ostomy/ Continence Management (WOCN) / (WCN).

Unit Specific Competencies: Must have clinical knowledge, skill and ability to fully and accurately complete all required unit competencies, as indicated in the unit competency check-list.

Physical Demands:
Job function requires extended standing or walking. Must be able to carry objects weighing 21 to 50 pounds. Able to lift or move objects weighing 51 to 100 pounds with the use of appropriate devices. Jobs in this group require frequent visual acuity to perform activities related to extended use of computers. Work Environment:
Jobs in this group are required to function in a fast paced environment with occasional high pressure or emergent and stressful situations. Frequent interaction with a diverse population including team members, providers, patients, insurance companies and other members of the public. Function is subject to inside environmental conditions, with occasional outdoor exposures. Exposure to various environments such as: communicable diseases, toxic substances, medicinal preparations and other conditions common to a hospital and medical office environment. May also be exposed to needle sticks, airborne infections, medical gases, X-Ray, chemical exposures and other potential hazards. Must wear Personal Protective Equipment (PPE) when exposed to infectious/clinical hospital environment.
