Overview:
The Youth Transition Coordinator provides mentoring, information and referral, advocacy, skills training and other supportive services to young adults, aged 14 to 22, with disabilities through our Pre-Employment Transitional Services (Pre-ETS) program. The Youth Transition Coordinator participates in transition planning to help young adults with disabilities plan for life after high school, enter the adult human services system, and live independently. This position offers flexible weekday hours.
Responsibilities:
- Conduct intake and assessment with students to develop pre-vocational and employment transition plans
- Collaborate with local school systems to deliver cooperative vocational education and training
- Provide career transitional counseling and supports for persons served
- Partner with community employers to establish work opportunities and paid internships
- Provide vocational exploration opportunities and skill-building training
- Assist individuals supported to understand their rights and responsibilities in the world of work
- Treat all individuals with respect and dignity and in an age and culturally appropriate manner
- Bachelor's degree in Rehabilitation Counseling, Counseling Psychology, Social Work, Special Education or related field.
- Three years of experience working with individuals with development and intellectual disabilities (highly preferred)
- Valid driver's license and ability to travel locally required.
