Summary

Coordinates all aspects of the preadmission process. Assesses all new referrals to establish financial viability. Serves as the liaison to referral sources from the community, hospitals and other referral sources such as Personal Care and Community. Provides tours and initial information sessions with family members. Coordinates the transfer process of residents between Presbyterian SeniorCare Network communities. Prepares daily admission, transfer, and ADT reports accurately and timely. Actively involved in marketing strategies for Presbyterian SeniorCare Network.

Essential Duties and Responsibilities include the following: Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Maintains compliance with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and community standards. Demonstrates on a consistent basis individual and team behavior that supports the Person-Centered, Woodwell and Woodside philosophies of Presbyterian SeniorCare Network. Consistently upholds the mission, values and behavior standards of Presbyterian SeniorCare Network. Maintains confidentiality. Other duties may be assigned.

Serves as the hospital liaison and assures positive communication with the hospital and promotes collaboration.

Receives referrals from ECIN, telephone calls, etc., and completes financial assessment to ensure appropriate placement and level of care.

Obtains appropriate authorizations as required.

Maintains budgeted census and evaluates for appropriate case mix.

Obtains clinical information and assures proper equipment and necessary medications are communicated to departments and are available for admission.

Coordinates all aspects of the application process including completion of application documents, facility tour and initial information sessions to ensure that applicants fully understand the eligibility requirements, services available and costs involved.

Informs all departments as appropriate of a new admission to assure a smooth transition for our new residents and coordinates admissions with all departments including Housekeeping, Nursing and Rehab. Reviews appropriate admission documentation with the resident and/or family, and ensures admission paperwork is completed within 24-48 hours of admission.

Obtains required resident information and obtains signatures on all admission documents. Inputs all admission, transfer, discharge information along with financial class information into computer for proper billing and accounting records for resident.

Completes the daily census report showing what activity occurred as of 12:00AM the prior day.

Utilizes established referral tracking connection sheets to uncover inquirer’s needs.

Education and/or Experience

Associates degree from an accredited school or 6 months of relevant experience or a combination of education and experience.

Computer Skills

To perform this job successfully, an individual should have knowledge of Accounting software; Spreadsheet software and Word Processing software.
