This position will provide direct patient care and have the opportunity to work with Star Community Health Centers, Inc. leadership to develop the role of clinical pharmacy within Star Community Health Centers, Inc. Uses evidence-based and best practice criteria to achieve medication therapy and chronic disease management for individuals seen in the Star Community Health Centers, Inc. offices. This is an excellent opportunity for pharmacists interested both in direct patient care and education.
Establishes, implements, and evaluates medication management policies, procedures, clinical
care, and data collection for patients under the care of Star Community Health Centers, Inc.. The clinical pharmacist will collaborate with physicians, providers, other clinical pharmacists, and other clinical team members to deliver care while shaping our processes to improve the quality of care.

Please note the selected candidate and new hire will be an employee of Star Community Health Centers, Inc. and not St. Luke’s University Health Network. Star Community Health Centers, Inc., affiliated with St. Luke’s University Health Network, is an organization comprised of community-based clinics and sites providing healthcare services to our local populations. New hire(s) will be recruited and hired via St. Luke’s University Health Network, but will transition to become a Star Community Health Centers, Inc. employee following the commencement of new hire onboarding.
JOB DUTIES AND RESPONSIBILITIES
ESSENTIAL FUNCTIONS:
1. Conducts patient medication history review and formulates an individualized patient
medication plan which includes lifestyle modifications and addresses barriers to
adherence.
2. Provides an organized and comprehensive approach to consultations for medication
management including indications, medication selection, medication dosage, access to
medications, drug reactions, and drug interactions.
3. Responsible for physician consultation and recommendations.
4. Working with Star Community Health Centers, Inc. and SLUHN/SLCN leadership and in consultation with SLUHN pharmacy leadership, develops and maintains policies and procedures related to medication management based upon his/her clinical experience in our ambulatory settings.
5. Serves as a resource and liaison for implementation of drug management initiatives
across Star Community Health Centers, Inc. Working with physician leadership, develops protocols and pilots them to initiate, adjust, monitor, and recommend drug therapy and care management. Functions as a direct-patient-care clinical pharmacist in these initiatives.
6. Collects, analyzes, interprets, and forecasts prescribing habits based on current patterns
from medication utilization reports.
7. Analyzes event data, summarizes trends, and provides information to clinical and
administrative staff to help support decision making.
8. Collaborates with clinical and administrative staff to determine need for process or
system improvements related to medication utilization, including polypharmacy, opioid
utilization, value-based prescribing, and optimization of medication therapy for select
chronic conditions including heart failure, diabetes, hypertension, and COPD/asthma.
9. Maintains an active leadership role on select committees and workgroups identified by
Star Community Health Centers, Inc. leadership.
10. Opportunities to lead project teams to implement process and/or system changes

function effectively as a management team member.
12. Maintains confidentiality of all materials handled within Star Wellness to ensure the proper
release of information.
13. Complies with all applicable policies regarding issues of employee, patient
and environmental safety and follows appropriate reporting requirements.
14. Demonstrates Performance Improvement in the following areas as appropriate: Clinical
Care/Outcomes, Customer/Service Improvement, Operational System/Process, and
Safety.
15. Demonstrates financial responsibility and accountability through the effective and
efficient use of resources in daily procedures, processes and practices.
16. Complies with applicable policies regarding attendance and dress code.
17. Demonstrates competency in the assessment, range of treatment, knowledge of growth
and development and communication appropriate to the age of the patient treated.
OTHER FUNCTIONS:
1. Other related duties as assigned.
PHYSICAL AND SENSORY REQUIREMENTS:
Sit up to 7 hours per day; 1 hour at a time. Stand for up to 8 hours per day; 4 hours at a time.
Walk up to 6 hours per day; 10 minutes at a time. Hearing as it relates to normal, high and low
frequencies. Seeing as it relates to general, far, near, color and peripheral vision; normal depth
perception.

Utilizing critical thinking skills to coordinate patient care and to interact with physicians, clinical
staff and administrative leaders in the development and implementation of clinical pharmacy
programs.

COMMUNICATIONS:
Excellent written and verbal communication skills required for contacts with physicians and
administration. The Clinical Pharmacist, Clinical Integration will regularly collaborate with
medical staff and other network departments.
ADDITIONAL REQUIREMENTS:
1. Doctor of Pharmacy (Phar.D.) degree
2. Proficient at working with Electronic Health Records Systems.
3. Working knowledge of the use of enterprise data warehouses and patient registries.
4. Computer skills including Microsoft Word, Office, Excel, and PowerPoint
5. Travel across the geographic area in which Star Community Health Centers, Inc. has a footprint for meetings and to work with clinical staff is anticipated. Occasional longer distance travel for a state or national meeting is possible. Current driver’s license and vehicle access required.
6. Works independently to meet expected deadlines for reports, projects and presentations.
7. Must possess excellent organizational skills.
8. Understanding of the principles of clinical integration and population health management
9. Active Pharmacy license in Pennsylvania by start date; eligible for licensure in New
Jersey
10. National Provider Identifier (NPI) is required.
MINIMUM QUALIFICATIONS
EDUCATION:
Doctor of Pharmacy degree is required.
TRAINING AND EXPERIENCE:
1. 3-5 years of clinical experience required. Management experience required.
2. Successful completion of a PGY2 pharmacy residency in ambulatory care or equivalent
clinical experience in direct patient care is required.
3. Excellent communication and project management skills including process improvement
preferred.
4. Experience with the development of clinical pharmacy programs within a clinically
integrated network or an accountable care organization.
