Position Summary:
The Memory Caregiver Aide operates under the Direction of the Director of Strategic Initiatives to assist with intake/referral of new memory care family caregivers, back-up case management activities, data collection, and outreach duties. This position is professional and requires the employee to exercise appropriate discretion and judgment in the performance of his/her assigned duties and responsibilities.
Qualifications:
EDUCATIONAL REQUIREMENTS
Associate's Degree in Human Services related field or Experience in Lieu of Education - experience serving older adults in a health care or service setting
EXPERIENCE REQUIREMENTS
customer service experience
SKILLS AND KNOWLEDGE
Excellent verbal and written communication skills. General knowledge of issues impacting older adults, healthcare issues and dementia.Ability to work independently.Knowledge and expertise with computer software applications and Word processing programs. Must enjoy working with the elderly.
Essential Functions:
- Provides phone support for incoming calls, new client intake and contacts with existing clients.
- Develop an expertise on Alzheimer's Disease and related dementias.
- Assist with client support groups and Memory Cafes
- Documents phone calls and case note phone calls in online case management systems
- Creates and maintain client list for each case manager.
- Assist with classes and public presentations
- Learn the Star-C evidence-based intervention the team utilizes.
- Provides clerical and data entry support as needed
