General Mental Health and Substance Use Case Manager
Teri's Health Services is an integrated healthcare company providing services to facility, in office, home, and mobile in the community. We provide services primarily to substance abuse and mental health co-occurring clients. Skills you must have include history of working with this population at minimum 1 year, have a solid work history and educational history showing you are ready for this type of position, be able to manage a high stress environment, be able to work on your own and at times (after training) without direct physical supervision. You will always have supervision and availability to, however, being that we are all mobile in the facilities ability to manage self, tasks, and develop your own agenda is a must. You must be professional, able to communicate well, and be willing and able to take feedback, correction on tasks, and manage a caseload.

Job Description:
- Work with a culturally diverse group of adults with mental health and substance use needs
- Support and advocate for the client’s needs and best interests.
- Educate and assist clients in obtaining available community resources as well as provide support to clients by transporting or coordinating for their goals to be completed.
- Communicates clients’ progress by conducting monthly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
- Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches.
- Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- oversee referrals from agencies.
Ideal Candidate Qualifications:
- (preferred) Bachelors or Masters degree in Human Services, Social Work or related field.
- Minimum 1 year of related experience

- Must be 21+
- CPR/First aid
- Level one fingerprint clearance '
- Gas Mileage reimbursement provided
