Position Description
The Care Coordinator-BSW provides guidance and education to patients and families and is responsible for assessing the psychosocial needs and coordination of care for a designated population of patients. Exhibits a deep understanding of each patient?s medical needs, insurance coverage, health literacy, and other socioeconomic factors that may impact their opportunity for wellness. Aids and assists patients in overcoming any barriers presented by these elements.
Position Requirements
Bachelor's degree in Social Work required upon hire. Master's degree in Social Work to be obtained within 2 years of hire. Excellent assessment skills. Strong organizational skills. Ability to work independently and team setting. Current American Heart Association or American Red Cross Basic Life Support (BLS).
Preferred Requirements
2 years or more of experience in healthcare or hospital setting coordinating post acute discharge services.
