We are seeking a Blended Case Manager to join our team at our program in Warminster, PA.

Job Description

The Blended Case Manager will demonstrate a Rehabilitation/Recovery attitude toward service delivery by providing consumer-driven strength-based approach to case management and crisis services. They will act as the primary case manager for assigned consumers as demonstrated by management of primary responsibilities including minimal bi-weekly support contacts at the client's home or wherever most appropriate, completion of consumer-driven service and safety plans, coordination of benefits, housing, and all other aspects of service delivery. If unable to contact the client, attempts will be made to locate them through family, friends, or relatives.

Earn $22.90 with the selection of the Enhanced Pay option.

Requirements:

Our ideal candidate will possess the following skills:

  • Bachelor's degree is preferred, but can hire:

    High school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthology, or other social science AND two years' experience in direct contact with mental health consumers.

    OR

    A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989

  • Valid Driver's license required
  • All clearances required by OMHSAS regulations
  • Spanish speaking is a plus!
