Summary*
This position will work as part of a team that utilizes a Harm Reduction* and Housing First Model to assist people by meeting them where they are, with housing as a primary focus. Team members will respond to each participant’s unique needs and goals by providing on-going support, advocacy and assistance in accessing community services to ensure stability in housing and integration in the community. Case Managers must be knowledgeable about and able to respond effectively to the complexities of participants’ lives and the co-occurring challenges and conditions associated with long-term homelessness.

Essential Responsibilities + Expectations*

Provide relevant support to participants to obtain and maintain housing: *
- Lead persistent outreach to potential participants building trusting and respectful relationships.
- Network with other agencies and individuals to help access housing and other needed resources.
- Support participants through issues of renter expectations, renter/landlord conflict resolution, and tenant rights.
- Assist clients with completion of necessary forms to access and maintain housing, including rental assistance and other benefits.
Provide high-quality case management service and on-going crisis intervention to participants:
- Manage a caseload of 15-20 participants.
- Offer weekly contact-focused on housing stability and participants’ goals.
Complete accurate and thorough documentation and recordkeeping in a timely manner:
- Maintain individual files, including case notes, signed data privacy, intake and discharge summaries, and Release of Information.
- Understand and comply with necessary documentation for MN Housing and Hennepin County Housing First/Housing Support (formerly known as GRH).
Provide positive representation of SHS through public relations and networking:
- Communicate and coordinate services with all team members. Attend regular team meetings and other required meetings.
- Interpret and promote the program to constituents, speak to community organizations or volunteer groups.
- Develop and maintain positive working relationships with staff, volunteers, participants and other stakeholders.
- Participate in professional meetings, workshops, and staff meetings.
Commitment to racial equity and social justice:
- Work within and across agency departments to build a culture of equity and anti-racism and hold SHS accountable.
- Evolve knowledge and strategy of equity and anti-racism on an individual and a team level.
- Participate in meetings, trainings, and workshops hosted by SHS about diversity, equity, inclusion, anti-racism, and other related topics.

This position involves significant time in the field, directly interacting with participants. This role is most suitable for someone who is confident in a public-facing role, interacting with people- many of whom are resilient but who have experienced trauma and face barriers because of their mental, physical and chemical health- regularly and in meaningful ways.

Job Requirements: *
- Bachelor’s degree in human services or comparable, relevant experience in the field.
- Three or more years of experience working with single adults who have experienced long-term homelessness or personal experience with homelessness.
- Understanding of and aptitude to apply knowledge of domestic abuse, childhood trauma, grief and loss, sexual violence and exploitation, homelessness, social support systems, advocacy, tenant law and case management in terms of providing services to women in crisis.
- Experience supporting people with mental health and chemical health issues.
- Computer proficiency, and ability to adapt to skills and programs as necessary.
- Valid driver’s license and daily access to a working vehicle.
- Awareness of boundaries and personal safety during home visitations.
- Comfortable and confident working both independently, but also in collaboration within a team.
- Ability to be flexible and manage time.
- Knowledge of Harm Reduction/Housing First Model and demonstrate commitment to that model.
- Knowledge of trauma-informed service and motivational interviewing techniques.
- Ability to communicate effectively, positively and professionally with participants, providers, landlords, family members and others.
- Creativity and confidence in problem solving and facilitating non-traditional tasks to meet participant needs.
- Ability to promote a professional work environment that is affirming, respectful and understanding of diverse people.
- Ability to maintain confidentiality.

- Someone who is confident in navigating and problem-solving throughout intense circumstances and challenging conversations.
- Very broad knowledge of housing resources in the Twin Cities Metro area.

Essential Physical Functions
The employee must be able to read, write, hear, and speak at a proficient level of English. Employee must be able to move about the community and public places (such as apartment buildings). Employee must be able to lift items up to 40 pounds on an occasional basis. Employee must be able to drive a van.
