The Client Care Coordinator (CCC) delivers on our mission to change the way the world ages by making positive, powerful impacts on clients, their families and our community.

We provide the BEST training for client care coordinator in the industry. We train you in our proprietary Balanced Care Method and Cognitive Therapeutics Method.

Client Care Coordinators (CCC) are primarily responsible for support and training of caregiving staff through a variety of activities including both in office and client home settings . Lead caregivers are also responsible working in the field as a caregiver on new clients starts or when the need arises .

  • Provide direct client care by filling in on assignments , which includes but is not limited :
  • Assistance with exercise , Activities of daily living (bathing , personal hygiene, dressing, etc .) Household services essential to a healthy home environment (meal prep, light housekeeping ,personal errands laundry etc.) Functions within the limitations of individual assignments .
  • Provides companionship and socialization .
  • Assist with meal planning , purchase of food ,and food preparation .
  • Assisted to live in a clean , safe environment
  • Knows follows infection control policies including proper handwashing techniques.
  • Maintain appropriate communication with the Administrator , Client Care Manager, Supervisor , or Director of Operations (written verbal).
  • Assists in the access to and documentation of medications that are self - administered by the client .
  • Reports any observed or reported changes in the client's condition and/or needs to Administrator, Client Care Manager , Supervisor , or Direct of Operations .
  • Follows the reporting guidelines on the individualized Service Plan .
  • Documents care provided and completes the forms required for the client's record .
  • Completes the appropriate records to document care given and pertinent observations .
  • Submits documentation per Agency requirements .
  • Promotes personal safety and a safe environment for clients by observing infection control policies , following Agency guidelines and reporting unsafe situations of the Administrator , Client Care Manager , Supervisor ,or Director of Operations .
  • Demonstrates safe practice in the use of equipment . Does not use equipment until orientation about its operation has been provided .
  • Notifies supervisor of educational needs .
  • Maintains confidentiality in all aspects of the job . Attends in- service programs to meet compliance requirements .
  • Identifies learning needs to Administrator , Client Care Manager , Supervisor , or Director Of Operations .
  • Performs other related duties and responsibilities as deemed necessary .
  • Treats clients and families with respect .

Lead new client start of care services

  • Introduction of caregivers
  • In home orientation / training of caregiver
  • Direct client care either with assigned caregiver or independently if caregiver not assigned .
  • Other start of care services as necessary to ensure quality client experience .

Caregiver field supervisor

  • In-home client caregiver visits to evaluate environment , work performance ,and client satisfaction .

In-home and in- office orientation /training of caregivers

  • Train caregivers on proper care techniques and company policies and procedures
  • Ensure caregiver is capable of performing care responsibilities in a safe and appropriate manner .

Caregiver Recruitment

  • Assist with recruiting activities including Job fair events and attending recruiting events .

Caregiving scheduling assistance

  • May need to assist the Employee Care Manager with scheduling caregivers too .
  • In office support including caregiver orientation training events , answering the phones greeting visitors , maintaining office including cleaning duties , inventorying supplies needed . This position may require you to travel up to 75% reimbursable local travel .
  • Schedule may vary day to day as priorities change .

The CCC must be available to our clients, referral sources and internal staff during regularly scheduled On Call rotations and as needed for nights, weekends and holidays. The CCC’s responsibility can be broken down into three main clusters – client acquisition, client retention, referral source marketing.

Required education:

  • BSW with 4 years experience or MSW or LVN with 4 years experience or RN

Preferred experience:

  • Home Care or Health Care related field: 3 years
  • Customer Service: 3 years
  • MSW: 1 year or RN: 1 year
