GENERAL DESCRIPTION :

Provide Clinical Pharmacy resource to physicals, nurses, patient and other healthcare staff.
-
Discharge medications counseling to patients.
-
Intervene in difficult CPOE orders by direct contact with physicians.
-
Round with physicians and other healthcare staff as appropriate.
-
Precept advance pharmacy students
-
Contact patients through continuum of care
-
Coordinate patient at home medication needs with care management
-

WORK EXPERIENCE REQUIREMENTS :
-
Previous hospital pharmacy experience
-
Residency preferred
-
-
EDUCATION REQUIREMENTS :
-
Pharm D or BS with residency or equivalent clinical experience
-
Maintain all CE requirements
-
Hold current license in State of Massachusetts
