Job Title: CRNP Hospice (face to face) visits
Location: Montrose, PA, Susquehanna County and surrounding areas
Shift: Per diem (must commit to orientation and training)

Position Details:
Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, evaluates hospice eligibility, and consults with the hospice interdisciplinary team. The nurse practitioner builds from the resources of the community to plan and direct services to meet the need of individuals and families within their homes and communities.

Responsibilities:

• Conducts physical assessments of patients as part of the re-certification of terminal illness process, per agency policy.
• Reports to the physician reactions to treatments, and changes in the patient's physical or emotional condition
• Maintains the highest standards of professional conduct regarding maintaining the confidentiality of patients at Traditional Home Health Care. Relays information only when patient's right to access is clearly established and sharing of such information is in the best interest of the patient.
• Maintains and enhances clinical knowledge and skills by attending, participating in, and/or conducting staff development and continuing education programs.

Experience:
•
Having experience with hospice patients or in the hospice environment is a plus.

• Understanding of hospice philosophy, principles of death and dying
• Must have Pennsylvania CRNP License
