The integration of self-management support in the delivery of healthcare to people living with chronic health conditions is a feature of the shift left from a model of dependency to one of collaboration between healthcare provider and service user. CHO DNCC is leading on the establishment of three Chronic Disease Ambulatory Care Hubs that will provide access to specialist services in the community for individuals living with more complex chronic diseases. These hubs will house a multidisciplinary specialist team to provide integrated, holistic assessment and service and will act as a single point of access to a wider host of services within the community for individuals with more complex needs. The recruitment of these specialist teams is currently underway.

CHO DNCC will play a critical role in the continuum of care from health promotion, disease prevention, diagnosis, treatment, disease management to rehabilitation services. The hub approach aims to improve both service user experience and the health outcomes for those living with chronic disease within a population of 150,000 -200,000. The hubs will be linked with 3 acute hospitals in CHO DNCC; the Mater, Beaumont and Connolly.
