Job description
As an Outreach Specialist at Aspire Health Partners, your job purpose will be to provide a multitude of essential functions regarding the coordination of services regarding reintegration into the community of person who are homeless. These functions include – but are not limited to – providing outreach services, such as, linking them to entitlement and housing, conducting assessments and completing referrals, assisting clients with benefits and negotiating the criminal justice system and advocating for diversion when appropriate, adhering to contractual obligations, serving as case manager and acting representative to committees and community planning and completing required training and participated in required meetings. Most importantly, all functions must be completed while maintaining high standards of ethical and professional conduct while adhering to agency policies and procedure
Requirements:- Level II Background clearance
- Acceptable Motor Vehicle Registration driver's license record in accordance with the underwriting guidelines set by Aspire insurance company as described in the AHP Corporate Transportation Policy 3.9.1
- Bachelor Degree Required
