Job Summary: This position is responsible for developing, implementing, and providing on-going oversight for the care coordination activities within Saint Francis Health System’s Population Health Services Organization (PHSO) to ensure identified patients receive high quality, efficient, and well-coordinated care. The position maintains responsibility for of all care coordination staff, including but not limited to Nurse Care Managers, Social Workers, Community Health Workers, and Care Coordinators. This position is accountable for delivering care coordination programs with measurable outcomes, while identifying and scaling best practices across the organization. The role works effectively in a matrixed reporting environment and collaborates closely with PHSO and Warren Clinic leadership, physician leadership, and manage operational leaders throughout the health system. Supervises employees engaged in daily care coordination activities for the purpose of delivering improvements in patient quality and care outcomes. Directly supervises other registered nurses, care coordination staff, and operations which have complex safety requirements.
Education: Has completed the basic professional curricula of a school of nursing as approved and verified by a state board of nursing, and holds or is entitled to hold a diploma or degree therefrom.
Licensure, Registration and/or Certification: Valid multi-state or State of Oklahoma Registered Nurse License or Licensed Practical Nurse. Current CPR Certification.
Work Experience: Minimum of 5 years healthcare and care coordination experience, including 3 years acting in a supervisory level position overseeing nursing staff.
Knowledge, Skills and Abilities: Working knowledge of care coordination delivery practices and physician practice operations. Knowledge of basic fiscal management and human resource management practices. Must utilize effective interpersonal, written and oral communications skills. Ability to organize and prioritize work and direct others accordingly. Ability to be detail oriented as required in the examination of clinical and numeric data.
Essential Functions and Responsibilities: Oversees the daily operations of the ambulatory care management programs and staff. Serves as a role model and mentor to all staff. Has the authority to interview, hire, orient & train, manage performance, conduct performance evaluations, and terminate staff. Builds and develops a workforce and team that delivers improved quality and effectiveness in healthcare outcomes for patients enrolled in the programs. Assigns day-to-day work activities, directs the functional and technical job performance of team members. Allocated and directs staff to meet patient and organizational needs. Develops and implements care coordination programs with the ability to measure program effectiveness and outcomes. Developed, implements, communicates, and oversees policies and procedures regarding the care coordination operations. Serves as clinical advisor and leader in interactions with health plans, employers, physicians, and provider office management & staff. Proficiently uses Epic and other computer applications to enable improved patient care outcomes with efficient and effective staff workflows. Effectively collaborates with technical resources and serves as the business owner to develop and supply reporting that demonstrates effectiveness in care coordination programs.Appropriately communicates and seeks counsel from Manager or other resources regarding operational issues. Builds and maintains strong professional relationships with leadership and physicians throughout the health system to enable attainment of organizational objectives. Leads quality initiatives and safety functions. Leads activities to promote employee engagement.
Decision Making: The carrying out of non-routine procedures under constantly changing conditions, in conformance with general instructions from supervisor.
Working Relationships: Direct Supervision of others - number of people supervised: (Varies) Prepares and gives performance evaluations. Works directly with patients and/or customers. Works with internal customers via telephone or face to face interaction. Works with external customers via telephone or face to face interaction. Works with other healthcare professionals and staff. Works frequently with individuals at Director level or above.
