We are seeking a Nurse Practitioner who will be accountable for a "defined" patient population. Functions in the roles of expert practitioner, consultant, educator, patient advocate, care coordinator and researcher within a specialized area of practice. Possesses and exhibits a high degree of knowledge, skill and competence in the provision of health care directly to the patient, or indirectly through guidance of care by their primary nephrologist. Provision of patient care is conducted and performed utilizing the medical model within the scope of licensure and in accordance with applicable law, regulations, and in accordance with The Nephrology Groups policies and procedures.

Upon acceptance of an offer of employment and prior to providing patient care, candidate must submit Scope of Practice and all information for credentialing, to our credentialing team.

II. Duties and Responsibilities:

  • Perform enhanced annual wellness visits or Continuous Health Evaluations (CHE) to identify and evaluate patients’ co-morbid conditions and identify and assist with patient’s’ needs including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members.
  • Manages the care of CKD/ESKD patients, using the enhanced annual wellness visit or CHE as a foundation history and physical, focusing on medical cost/utilization and preventing/reducing hospitalizations, and re-admissions through proactive post-discharge planning and follow-up with patients most at risk.
  • Leads the clinical team and ensures care management and care coordination activities complement and support medical treatment plans while integrating the psychosocial, and physiological needs of the patient.
  • Documents findings in the patient medical record using an Electronic Health Record (HER) system, ensuring that appropriate diagnoses are evaluated and accurately documented.
  • Communicates and collaborates with nephrologist, other providers, clinical teammates,
  • IKC teammates, Medical Director regarding the healthcare needs of patients.
  • Knows and complies with policies and procedures in accordance with State and Federal regulations necessary to maintain organizational accreditation.

III. Qualifications and Requirements:

Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease.

Ability to become credentialed.

Deals with confidential information and/or issues using discretion and judgment.

Functional proficiency with HER applications within 90 days of employment

Intermediate computer skills and proficiency in MS Word, Excel, and outlook

Education/Training/Certifications:

  • Master’s Degree
  • Current NP license; Certification completion from an accredited NP Program
  • Current CPR certification
  • Current DEA license or eligibility to obtain within 90 days of hire
  • Current valid drivers license and insurance with in the state of residence

REQUIREMENTS

Active CA RN license. Graduation of a Nurse Practitioner program and a Master's degree. Nurse Practitioner National Certification is required within six (6) months of hire or transfer into position. Experience Maternal Fetal specialty highly desirable. A minimum of 1 year related experience is required and 5 years Increasingly responsible experience which demonstrates professional working knowledge of nursing theory and practice. Basic Life Support (BLS) is required within 30 days of hire or transfer into position. Must successfully complete the credentialing process through our credentialing team, written standardized protocols, and furnishing privileges. Ability to work independently, problem-solve, and assume responsibility for inpatient/outpatient care. Strong organizational and time management skills. Excellent interpersonal, oral/written communication skills for effective interactions with patients and their family members, and colleagues in multiple medical disciplines.

QUALIFICATIONS

  • Excellent communicator with staff, physicians, patients and their families
  • Professional appearance and attitude
  • ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred)
  • Active and current Physician Assistant or Nurse Practitioner licensure in the state of California, DEA number, and state-controlled substance certificate, as required
  • Department of Transportation (DOT) certification preferred.
  • Able to multi-task and work independently
  • Full Time - Exempt level position
  • Office clinic open Monday-Friday and closed on weekends and holidays
  • Currently call is not required but may be asked to cover dialysis rounds if needed
