Position Summary

Under the supervision of the Regional Director, the Placement Coordinator will help raise public awareness of the agency's mission and issues related to care of individuals with IDD to help enlist community organizations, churches, educational institutions, businesses, and individuals to support the agency through referrals and volunteer activities. The Placement Coordinator is primarily responsible for organic census growth as well as helping cultivate ongoing community support for the agency. The Placement Coordinator will help the agency build a network and database of community contacts, resources, and supportive volunteers.

Essential Duties and Responsibilities

  • Build relationships with a network of community partners such as LIDDAs, PASRR Placement Coordinators, ICF Placement Coordinators, HCS Services Coordinators, SSLCs, Nursing facilities, Schools, Guardianship Agencies, CPS, Parent Groups and Advocacy organizations to develop resources that support the agency's organic growth as well as advance overall company goals.
  • Coordinate and manage all educational, marketing, admissions, and sales activities.
  • Maintain a complete database of current and potential referrals and community partners using agency software.
  • Help develop strategies through networking and engaging community members to help increase organic growth and public relations at the agency.
  • Attend community functions and activities designed to introduce the public to the agency and recruit host home providers.
  • Participate in planning and execution of special events as needed.
  • Serve as a liaison to the community by speaking to individuals and organizations who are interested in learning more about getting involved with the agency.
  • Responsible for recruiting, planning, directing, coordinating, organizing, and implementing the systems, processes and programs needed for the intake and admissions of potential and new individuals.
  • Perform all job responsibilities in alignment with the mission and core values of Caregiver, Inc., and comply with all policies and procedures, OSHA, CMS, HIPPA, local, state, and federal, regulations.
  • Responsible for all aspects of the referral management process including but not limited to receiving and tracking all referral sources.
  • Perform all other duties as assigned by management.

Education and/or Experience

  • Bachelor’s Degree in Social Services, Business or Sociology preferred.
  • Demonstrated business development performance also preferred.
  • Minimum of 5 years’ prior experience working with children or adults with developmental and intellectual and/or physical disabilities preferred.

Knowledge and Skills

  • Excellent written and verbal communication.
  • Bi-lingual is preferred.
  • Ability to work in a professional environment.
  • Extensive knowledge of HCS and ICF regulations, especially as they relate to admissions and service array.
  • Established relationships with referring agencies is preferred.
  • Ability to handle multiple tasks efficiently and effectively.
  • Ability to organize and coordinate public speaking events and presentations.
  • This position requires a valid driver’s license, a good driving record, and a registered/ insured vehicle.
  • Must complete all required training as assigned.

Pay: $65,000.00 - $70,000.00 per year
