The Care Coordination Program Supervisor provides clinical and administrative supervision of a team if Care Manager Specialists and is responsible to oversee and ensure delivery of high quality services to qualified Medicaid recipients enrolled in the St. Joseph’s Medical Center Care Coordination program. The program supervisor is responsible to lead and monitor practices to include face-to-face, telephone outreach, assessments, Plans of care, intervention, referral, and linkage to services. The Care Coordination Program Supervisor complies and maintains programmatic data reports, works with Health Home staff and health providers to ensure positive health outcomes for all clients. The supervisor reports to the Administrative Director of Care Coordination and is responsible to ensure timely and accurate data, submissions and record keeping.

Job Type: Full-time

Pay: $45,901.19 - $72,519.12 per year
