5 -10 years Clinical Pharmacist experience in a hospital setting

General Summary:

Under the general supervision of the Regional Director of Pharmacy Service (east), and the Senior Director-Pharmacy Services, and as a member of the Institutional Clinical Pharmacy Leadership Team (ICPLT), is responsible for supervising and directing all aspects of clinical services for the central region, which includes developing, monitoring, evaluating and coordinating services that promote evidence-based use of medications, financial sustainability and positive patient outcomes for hospitalized patients throughout Wellspan Health via proper medication usage.

Duties and Responsibilities:

1. Identify the need for provision of clinical services across the system based on patient care requirements while identifying resource, training and educational gaps impacting provision of services

2. Preparation, implementation, and review of clinical standard work in support of P&T approved protocols for medication therapy management

3. Support educational needs for pharmacists across the system by identifying continuing education opportunities, in-services, and conference attendance

4. Review and recommend drug information resources in support of clinical pharmacy services

5. Create and support training opportunities within WellSpan for pharmacists to engage and learn clinical practice areas not readily available at their present facility, including protocol and consult services

6. Identify and direct appropriate clinical pharmacist support of system wide committees and initiatives

7. Provide regional leadership in the evaluation and implementation of local and national GPO initiatives

8. Develop, measure, and analyze, on an annual basis, metrics of clinical pharmacy practice and interventions on improving outcomes, adverse event avoidance and cost savings

9. Provide regional leadership in the development and enforcement of a system wide formulary intended to provide optimal therapeutic options for all patients while eliminating duplicative and unnecessary medications.

10. Identify and support changes to the EMR to enhance opportunities to identify potential patient related interventions, documentation, and possible 3rd party vendor product replacement.

11. Members of the ICPLT will share responsibilities for system wide committees including the Pharmacy and Therapeutics (P&T) and Order Set Committees, and the Institutional Review Board (IRB).

Qualifications:

Minimum Education Doctor of Pharmacy degree or equivalent advanced degree in pharmacy

Minimum Experience 5 - 10 years as a clinical pharmacist in a hospital setting and preferred experience as a clinical coordinator or clinical manager

Req Cert/Registration RPh and current PA licensure

Requires judgment/action which could result in death of patient
