RN Care Manager/Registered Nurse Care Manager Job Responsibilities:

• Completes discipline-specific initial and ongoing assessments. Conducts home visits and home assessment as needed, and consults w/ families and other supports for additional information as needed.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports. Follow up on all services to ensure quality/coordination and effectiveness of services.
• Complete all required documentation and charting and assists in the development and maintenance of member records.

RN Care Manager/Registered Nurse Care Manager Job Requirements:

• Education: Associates degree in Nursing Required, Bachelor’s Degree preferred.
• Licensure: Wisconsin Licensed Registered Nurse
• Licensure: Eligibility for long-term care functional screener certification is required
• Licensure: WI Driver’s License
• Experience: Required 2 years recent experience with older adults or person with physical or intellectual disabilities in a nursing capacity; Preferred 2 years of work in community nursing

RN Care Manager/Registered Nurse Care Manager Job Schedule:

• Monday to Friday-8:00 AM – 4:30PM
