Integrated Care Programme for Older Persons

There is currently one permanent part-time (17.5 hours per week) vacancy available, based at Day Hospital, St. John’s Hospital, Enniscorthy, Co. Wexford.

A panel may be formed as a result of this campaign for Social Worker, Senior Medical for

South East Community Healthcare (SECH) Integrated Care Programme for Older Persons (ICPOP) from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled.

Details of Service:

The Integrated Care Programme for Older Persons (ICPOP) seeks to support older persons to live in their own community by providing timely access to health and social care that allows older persons to receive the right level of care in an appropriate location. The key objectives of ICPOP are to:

• support older adults to live well;

• enable older adults to remain in their place of residence by providing secondary specialist care in the community;

• provide integrated Intermediate Care (Hospital/Community Care).

Within the overarching ICPOP programme, the Memory Assessment & Support Service (MASS) provides a holistic diagnostic and post-diagnostic support service to adults experiencing cognitive decline.

“The Memory Assessment & Support Service is a space where a comprehensive assessment is carried out by the multi-disciplinary team (MDT) where there is concerns about suspected dementia or a mild cognitive impairment. Importantly the MASS also provides post-diagnostic support and follow up through (i) a specialist dementia post-diagnostic service and (ii) a brain health service; with a comprehensive care and support plan developed for each individual….

The MASS post diagnostic service will provide personalised assessment, risk reduction intervention, care planning, referral PDS intervention and arrange a point of contact and follow up. In line with the Draft Model of Care for Dementia in Ireland, for disclosure of a diagnosis of YOD or complex presentation the disclosure of a diagnosis will be managed by:

• Consultant, Specialist Registrar or Registrar, plus

• Senior Social Worker.

For disclosure of a diagnosis of dementia to those over the age of 65 at Level 2 – MASS, disclosure will be managed by

• Consultant, Specialist Registrar or Registrar, plus one of the following:

• CNS/ANP

• Occupational Therapist

• Social Worker

• Speech and Language Therapist (depending on diagnosis)

• Psychologist

The MASS teams are currently under the clinical governance of the acute Hospital Consultant Geriatrician and consists of Advanced Nurse Practitioners, a Senior Clinical Psychologist, an Operational Lead and an Administrator. Recruitment is underway for additional team members including a Senior Medical Social Worker, Senior Occupational Therapist, Senior Dietitian and Senior Speech & Language Therapist.

Purpose of the Post:

The purpose of the post is to provide a comprehensive social work service within a multidisciplinary context and in so doing, ensuring a person centred service to individuals and families.
