Shift Available:
- PRN
Responsibilities:
- Provides therapy services to residents and initiates referrals when appropriate and in accordance with Rehabilitation Department procedures
- Review requests for Speech Language services and physicians’ orders
- Decide on type of therapy most desirable after consulting with physician
- Conduct screening of residents at regular intervals to determine need for intervention/treatment
- Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated
- Interprets and communicates evaluation findings to residents, family, and other team members
- Establish individual resident schedules in conjunction with other team members
- Develops treatment plans by identifying a problem list, including short- and long-term goals and methods to achieve identified goals
- Participate in development and implementation of resident assessments (MDS) and care plans, including quarterly reviews
- Coordinates treatment plan with resident, family, and other team members
- Implements and conducts treatment as outlined in treatment plan
- Monitors resident's response to intervention and modifies treatment as indicated to attain goals in accordance with physician’s orders
Qualifications:
- Must possess a master’s degree in Speech Language Pathology.
- Must possess current, unencumbered license to practice as a Speech Language Pathologist from the Commonwealth.
- A physical examination and/or evidence from a physician indicating freedom of communicable disease to be available upon employment.
- Must be in good physical and mental health.
- Ability to work independently or part of a group.
- Effectively communicate with others.
