Position: Therapist, LCSW/LCPC
Department: Behavioral Health
Reports To: Director of Behavioral Health
Position Status: Exempt
Position Summary
The Therapist is a clinical staff member supporting our model of patient-centered, team-based care that integrates behavioral health with primary care, leading to improved patient health outcomes and overall satisfaction. This position is focused on SUD and Co-Occurring disorders treatment. The therapist will work closely with PCP’s and the Substance Use Disorder Care Coordinator in the Medication-Assisted Treatment program to provide behavioral health support. While the position will work from one clinic, the therapist will receive referrals from across PrimeCare Health. This position will also receive referrals from PrimeCare Health’s internal Long Term Therapy program.
Duties and Responsibilities
- The Therapist will provide substance use disorder/co-occurring disorder treatment to patients in an outpatient setting,
- Completes assessments and provides interventions/therapy to patients referred for substance use treatment, including patients receiving services through the Medication-Assisted Treatment (MAT) program.
- Conducts risk assessments and supports patients in crisis, as indicated.
- Maintains a visible presence during clinic operating hours.
- Coordinates care with team by responding in a timely manner to communication via phone, email, or Athena (EMR) text.
- Completes clinical notes and other necessary paperwork in a timely manner to comply with all administrative regulations. Completes visit notes within 72 business hours of visit completion.
- Educates the care team on basic strategies for supporting patients experiencing substance use/co-occurring disorders.
- Documents as directed to allow program evaluation and fidelity tracking.
- Attends required meetings regularly, including the Provider and Site meetings.
- Meets the productivity goal of 1200 annual billable visits within 1 year of starting with PrimeCare Health. If not meeting this goal, actively deploys strategies to get to goal.
- Therapist will assist in meeting expectations of the Behavioral Health objective in Health Resources and services administrator 330 grant.
- Other duties and responsibilities as assigned.
Required Skills or Abilities
- Highly skilled in providing behavioral health services with previous experience working in the substance use disorder field preferred.
- Strong interest in providing services to individuals, families and couples.
- Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
- Experience with Motivational Interviewing and Harm Reduction approaches.
- Experience and/or knowledge of substance use disorders, co-occurring disorders and trauma treatment, and working within a multi-disciplinary team.
- Proficient in the use of computers and Microsoft Office.
- Proficient in the use of an electronic medical record.
Physical Demands
- Must be able to remain in a stationary position 50% of the time.
- Must be able to move around the clinic site 50% of the time.
- Constantly operates a computer, computer printer, copy machine, and telephone.
- Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
- Must be able to transport from one site to another.
- Must be able to cover other shifts as necessary.
Required Knowledge, Experience, or Licensure/Registration
- Master’s degree in Social Work, Counseling, Psychology. LCSW or LCPC required.
- Must possess all licenses, registrations, certifications, and accreditations required by law to render health care services in the State of Illinois.
- Must maintain full credentials and privileges to practice with PrimeCare Health.
- Minimum of three years of clinical experience as a behavioral health provider in a health care setting required.
- Work flexible work schedule, which will include one evening per week.
- Previous experience in a primary care setting preferred.
- Previous experience/familiarity working with substance use disorders preferred.
- Strongly preferred bilingual in English/Spanish
