We at Fairview Health Services are seeking a driven Transplant Coordinator to join the hard working Liver/Post Transplant specialty team that is based on the University of Minnesota Campus. This role approved is for 80 hours every two weeks and is benefit eligible position! When it comes to adult-to-child, partial, full, and living-donor liver transplants, M Health Fairview Transplant Care leads the region with more than 1,500 procedures to date. Recently, the Scientific Registry of Transplant Recipients (SRTR) determined our Liver Transplant Program to have one of the most successful outcomes on survival after transplantation in the nation by SRTR criteria - 5/5 bars. A first for the program, with less than 10% of transplant programs in the US achieving the rating; and we were only program in the region to achieve this status!
Responsibilities/Job Description:
The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.
  • Coordinate patient care in both pre- and post-transplant phases
  • Act as a liaison for transplant physicians to implement medical management of conditions
  • Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and changes in patient conditions
  • Apply appropriate medical interventions and transplant protocols based on assessment, lab results, and patient condition changes
  • Assumes on-call responsibilities after office hours by acting as a resource for medical triage. Oversees' transplant coordination when donor organ becomes available.
  • Provides ongoing education to patients and their family
  • Collaborate with other clinical team members to improve patient care and benchmarks through quality improvement projects
Qualifications:

Required

  • BA/BS in Nursing
  • 3-5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg
  • MN RN license
  • BLS
