Reports directly to the designated Associate Director of Substance Use and Latinx Services. Clinician provides individual consults, triage, crisis, assessment and counseling to MAT clients. SCHEDULE: Full-time (40 hr/week) at two main CommuniCare clinic sites (West Sacramento and Woodland site) and in-community as needed.
RESPONSIBILITIES:
The following reflects managements definition of essential functions for this job but does not restrict the tasks that may be assigned. Management may assign or reassign duties and responsibilities to this job at any time due to reasonable accommodation or other reasons.
- Conducts in person screening/admission and assessments for adult MAT clients.
- Clinician is a member of the MAT team and is responsible for: responding to crisis, triaging client needs and conducting comprehensive biopsychosocial assessments for clients experiencing moderate to severe SUD symptoms and conditions, care coordination and referral, understanding billing structure, and how to correctly utilize and interpret ICD10 and DSM5 codes and criteria.
- Provides short counseling to MAT participants and consultation/education to family members if needed.
- Works as a member of a multi-disciplinary team including Behavioral Health staff and Primary Care. Collaborating directly with Prescribers, peer partners, supervisor, community stakeholders such as the Yolo County Jail.
- Monitors patient charts to ensure the timely placement of all correspondence, completes all screening and data collection tools, tracks caseload, and completes discharges according to protocol and best practices. Participate in quality assurance and improvement efforts.
- Supports in weekly medication inductions coordinating with MA and prescribing MAT Physician.
- Performs screenings, warm hand-off follow ups and makes appropriate referrals to SUD treatment programs within and outside CCHC.
- Completes all client information and documentation in a timely manner.
- Works with clients to establish long and short term goals. Counselor is responsible for detecting emotional, psychological, and physical danger signals. Counselor is non-judgmental and offers feedback and support to clients.
- Provides information to clients including treatment alternatives and referral service.
- Responsible for remaining within the ethical standards established in the field, including those of confidentiality.
- Responsible for explaining and enforcing all rules and regulations associated with clinic treatment policy.
- Compile and report statistics as required.
- Attend clinical supervision and weekly staff meetings. Attend training/conferences and performs presentations as arranged.
- Responsible for collaboration with CPS, Drug Court, Parole, Probation and other agencies to case manage clients as directed by supervisor.
- Responsible for maintaining the standards as listed in the CommuniCare Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position.
- Other projects as may be assigned.
MINIMUM POSITION REQUIREMENTS:
Education: MSW/AMFT preferred.
Experience: A minimum of two years experience working in a behavioral health/drug treatment program.
Special Skills/Equipment: Bilingual Spanish/English required. Must provide proof of current drivers license and auto insurance.
Expectations: Attendance and punctuality is necessary. Ability to work well with a multidisciplinary team. Ability to problem solve and be self-motivated. Understanding and use of DSM criteria. Computer proficiency. Must have a commitment to excellence and high standards. Must have excellent written and oral skills, strong organizational, problem-solving and analytical skills. Must have ability to manage priorities and workflow. Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and have acute attention to detail. Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions. Able to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers. COMPLETED COVID-19 VACCINATION REQUIRED
Physical Requirements: Required to sit for prolonged periods. Able to bend and able to lift 15-25 lbs. Hearing and vision are required.
PAY RATE:
- The pay range for this position is:
- AMFT/ASW/APCC: $66,560/yr.- $68,640/yr. (Based on 1.0 FTE)
- LMFT/LCSW/LPCC: $78,936/yr.- $95,000/yr. (Based on 1.0 FTE)
- Starting pay within this range will vary depending on the job-related knowledge, skills, and experience.
- May be eligible for annual 2% + 0-2% merit-based pay adjustment (0-4%).
