MultiCultural Home Care, a Modivcare Personal Care Service, is looking for a Scheduling Coordinator to schedule client visits with a caregiver, document communication between clients and caregivers, and ensure branch compliance with client-related documentation requirements. You will report to the Area Director or local Branch Manager.

You will...

• Responsible for coordinating Home Health Aide, Personal Care, Homemaker, Companion, and Heavy Chores services for clients as needed.
• Administer the Home Care Program to ensure the service is delivered appropriately, caring, and respectfully.
• Ensure that all services are provided as scheduled.
• Act as a liaison between workers, social workers, clients, and client's families to ensure proper care coordination.
• Ensure that care is provided according to all relevant policies, procedures, and regulations

• High School Diploma or GED
• 1+ years of home healthcare experience preferred
• 2+ years of customer service experience
• Bilingual in English & Cambodian/Khmer required

Pay: $22.00-$24.00/Hourly

Schedule: M-F 8:00AM-5:00PM & Rotating on-call required
