We are now hiring for a full-time Pathways to Success Career Coordinator to join our Southwest Center Team!

ESSENTIAL FUNCTION:

The Care Coordinator provides comprehensive care coordination as part of a new State Initiative that will enhance access to critical behavioral, medical and social services for children with complex behavioral health challenges in Illinois. Care Coordinators manage the care to clients, facilitate Child and Family Care Teams utilizing High Fidelity Wrap Around or Intensive Care Coordination

This position provides coordinated care to participants and facilitates the Child & Family Care Teams through the comprehensive High Fidelity Wrap Around (Evidence Based Practice) services or the Intensive Care Coordination service model. The position will be involved with the oversight and coordination relating to integrated care, development of individualized care planning (IM+CANS), participation in Child and Family Teams, and coordination with case managers and other caregivers.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provides community care coordination utilizing High-Fidelity Wraparound or Intensive Care Coordination service delivery model
  • Facilitates Child and Family Team Meetings for designated caseload
  • Conducts necessary assessments and develops collaborative, strengths based child and family service plans
  • Provides outreach and education to children and families about care coordination and eligible services
  • Conducts community engagement and provides services to the child and family in home and community-based settings
  • Gathers information for the IM+CANS and related care plans,
  • Monitors daily alerts for participants entering an emergency room or inpatient hospital processes and other coordinated care interfaces, following up with clients and facilitating post-discharge appointments.
  • Documents the care/service plans and other vital information in electronic health record after each contact and service.
  • Actively leads care planning process that specifies direct care resources to meet physical and psychosocial needs; by prioritizing problems and establishing mutually agreed upon goals specific to the client
  • Participates as assigned in participant education including development of materials, conducting presentations or supporting other team members in such efforts.
  • Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients
  • Maintains timely and accurate client records and any necessary agency and funder required program reports.
  • Participates in agency and site based required meetings trainings.
  • Other duties as assigned.

QUALIFICATIONS

  • Bachelor’s degree required, preferably in Human or Health Services (or meets 2 years of supervised clinical experience in a mental health setting required.)
  • Experience working with diverse child and family populations with significant behavioral health needs, social service experience is preferred.
  • Requires mobility to work across the region and attend meetings as necessary. Vehicle, valid driver’s license, and insurance
  • Bilingual preferred
  • Excellent oral and written communication skills, including written, social media, giving community presentations and proficiency in Microsoft Office Suite and Google tools.
  • Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
  • Ability to work effectively with diverse individuals and groups.
  • Self-starter that is detail-oriented and thinks creatively and strategically.
  • Ability to produce high-quality work independently.
  • Demonstrated commitment to social justice, community empowerment, and advocacy.

ADDITIONAL REQUIREMENTS:

  • MFS staff are required to provide proof of COVID-19 vaccination or have an approved Medical exemption prior to starting employment. If an approved Medical exemption is obtained, weekly testing is required.

PHYSICAL DEMANDS:

  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
  • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
  • The employee may occasionally lift and/or move up to 25 pounds.

SALARY:

  • Compensation is commensurate with years of related experience, position requirements, and candidate qualifications. The average salary for the position is $42,330.48.
