In our time of Company growth, we are seeking a Care Coordinator Navigator.

Work Location: Bradenton, FL

As part of this role, you will:

• Effectively works with patients, staff, health service providers, agencies, etc. from diverse backgrounds to reduce cultural and social-economic barriers between patients and institutions
• Receives assigned patients and maintains a patient caseload for select navigator positions consistent with program targets
• Clearly communicates the purposes and services available in the Care Coordination program to patients, family members and caregivers
• As part of the Care Coordination Team, assists patients in understanding care plans and other instructions and helps patients actualize health management plans and goals

What you need to bring to this role:

• High School diploma required
• Associates Degree in a health care or public health field preferred
• Medical Assistant or LPN preferred
• 1-3 years' experience in a healthcare or public health area with direct patient interaction required
• Excellent written and verbal communication skills
• Knowledge of Medical Terminology preferred
