Care Intergration Manager for the InCK Program

The Monmouth/Ocean County Care Integration Manager (CIM) provides Medicaid beneficiaries, Birth through 20, access to the NJ Integrated Care for Kids (InCK) Program. The CIM assists the beneficiary family in completing an Intake Needs Assessment and Interview and the development of an Integrated Care Plan (ICP). The CIM acts as a liaison to Pediatric Primary Care and Community Based Services ensuring Medicaid beneficiaries receive the services needs for optimal health and well- being.
The CIM utilizes the NJ InCK Data Base System to collect, develop and facilitate Integrated Care Team Person Centered Care.
Responsibilities:
The core responsibilities for the Care Intergration Manager, InCK Program, include:

  • Develops strong functional relationships with co-workers, learning from their expertise and gathering guidance from them as soon as onboarding begins.
  • Reports to and works with VNACJ Children & Family Health Institute’s Director of the Center for Early Childhood, Family Support and Integrated Care Coordination System of Service Delivery for Medicaid enrolled children and youth age 0 through 20 in Monmouth and Ocean Counties.
  • Establishes and maintains effective and collaborative relationships with the medical/behavioral health providers and community-based services in Monmouth and Ocean Counties.
  • Assists the family in accessing and completing the Needs Assessment.
  • Conducts a CIM/Interview and gathers information in order to complete an Integrated Care Plan and Care Team.
  • Reviews, analyzes and provides data driven information utilizing the InCK Report system to the InCK Core Team, weekly, monthly, quarterly and annually.
  • Tracks InCK Program performance and successful completion of outcomes for assigned children/youth, and ensures families are receiving goals from the Care Plan.
  • Adheres to VNAHG policies, including those for infection control, trauma informed resilience oriented care and safety.
Qualifications:
The ideal candidate for the Care Integration Manager position will have the following qualifications:

  • Health Related Degree from a four-year college or university
  • 5+ years of professional work related to the care of children and /or youth, prior care coordination and organizational management experience. Knowledge of chronic conditions and medical complexity in children.
  • Computer proficiency, data analysis skills, Microsoft Office including advanced Excel and Report Management
  • Excellent verbal, written and interpersonal communication skills
  • Strong organizational and team management skills
  • Bilingual Spanish highly preferred


Working Conditions/Physical Demand
: Working indoors and may be required to travel outside to business and community locations as needed.
