Job description

The Case Manager provides quality case management, psychotherapeutic services, and discharge planning to all patients and their families; to serve as a member of interdisciplinary team supporting the organization's treatment program and philosophy and assure the deliverance of quality treatment to patients and their families.

Education & Experience:

· Bachelor’s Degree in Social Work or related field required

· Master’s Degree preferred

· 3 years behavioral health experience preferred

· Associate license such as ASW, AMFT, APCC preferred

· Current CPR

· MAB to be obtained w/in 60 days of hire

Aurora Charter Oak Hospital has been delivering Behavioral Health Treatment Services for over 80 years. We are located on 10 acres of beautifully landscaped grounds in Covina, California. Covina is about 22 miles east of downtown Los Angeles, in the San Gabriel Valley, close to 10, 60 and 210 FWY.
