Case Manager
The Mental Health Association of San Mateo County (MHA), a nonprofit organization, enriches quality of life, restores dignity, and reduces homelessness, particularly for those affected by mental illness and HIV/AIDS. MHA strengthens hope by providing shelter, support and opportunities for socialization.
Definition of Position: Provides supportive case management services for individuals with psychiatric disabilities who reside in subsidized apartments in San Mateo County.
Specific Job Duties: Under the direct supervision of the Case Management Services Director the case manager is responsible for the following:
- Maintaining regular contact with all clients-at least two contacts a month with each client and at least one of the contacts being in person.
- Becoming familiar with other agencies and service providers in the county and developing good working relationships with these resources in order to provide clients with the services they need.
- Ongoing assessment of clients’ independent living skills, utilizing occupational therapy services, as needed, to assist clients in developing the skills they need to maintain independent living.
- Maintaining frequent contact with the Housing Authority of San Mateo County to assure that client is complying with the rules and regulations of the Permanent Supportive Housing, Aftercare, or Section 8 programs.
- Providing ongoing support for clients while continually assessing their need for increased mental health services and providing crisis intervention when needed.
- Documenting all billable contacts with clients on at least a weekly basis and filing documented notes in charts a minimum of every two weeks.
- All other clinical documentation including annual Client Treatment Plans and Annual Assessments completed in a timely manner.
- Frequent contact with supervisor to inform supervisor of daily schedule and general status of caseload.
- Regular participation in staff meetings and clinical consultations.
- Driving throughout the county is required.
- Must have a valid drivers license, working vehicle and clean driving record.
- Other tasks assigned as needed.
Qualifications:
- B.A. in Psychology or a related field.
- Two years experience working with people with psychiatric disabilities.
- Ability to be flexible.
- Ability to work with a diverse population.
- Good computer and documentation skills.
