Description/Job Summary
The Case Manager is an integral part of a multi-disciplinary team of mental health clinicians. The Case Manager delivers routine ACT treatment services such as wellness selfmanagement counseling, integrated dual disorder treatment, daily living skills, and general case management. This is a field-based position and requires at least 75% of services to be provided to participants in their homes, shelters, places of employment, vocational and education settings, and other community settings.Responsibilities/Duties
Attend ACT team daily meetings Provide education to team members regarding vocational/educational issues Develop recovery plans in collaboration with the participant and the participant's support system Involve family and other natural supports in the participant's treatment with the ACT team and provide psycho education to supports as needed Complete comprehensive assessment, crisis plans, progress notes, and other required paperwork as required Assess and provide supports to participants to address needs for problem-solving, wellness self-management, housing, education and vocational training, social supports, and employment. Act as an advocate and liaison for participants to seek and gain access to adult education, vocational training and competitive employment Provide 24-hour on-call services and crisis intervention, on a rotating basis Provide 75% percent of treatment contacts in the community Provide culturally competent services that are individualized to participant's needs and reflect the integration of their race, color, religious creed, disability, handicap, ancestry, sexual orientation, gender, gender expression, national origin, age and sex Maintain confidentiality and adhere to HIPAA Perform other duties as required by Team Leader/Unit DirectorRequired Experience
At least one year of relevant experienceRequired Education
At least Bachelor's Degree in a Human Service AreaRequired Qualifications
Fluency in Spanish preferred
CADC, CAADC or CCDP required
At least Bachelor's Degree in a Human Service Area
At least one year of relevant experience
Ability to pass a drug screen and background check Valid driver's license and car insurance
Highly organized andself-motivated
Proficient in computers

The Kent ACT team provides non-traditional community based mental health treatment to promote the recovery and community integration of 75 adults with serious mental illnesses and co-occurring substance use disorders living in the community to reduce use of ERs and inpatient hospital stays.
