Looking for a full-time Case Manager for our Division of Family Development department (DFD-ICM)
Job Summary:
You will provide care management services for Bergen County residents that are at risk of homelessness that promote maintenance of permanent housing, independence, and stability in the community.
What You Will Do:
- Assess clients at intake, during ongoing service delivery, and at discharge
- Collaborate with service providers to establish comprehensive assessments and care plans
- Coordinate continuity of care within the agency, community, jails, hospitals, etc
- Formulate comprehensive care plans containing relevant, measurable objectives and realistic time frames for achievement; perform quarterly reviews of progress and evaluate the effectiveness of goals
- Provide or link to health care services, dental care, benefits, mental health services, substance abuse services, family consultation, crisis intervention, and vocational, educational, or employment services; etc.
- Complete ongoing evaluations/reassessments through regular contact with clients to appraise changing needs and progress towards goals. Such contact should consist of home visits, client telephone conferences, client office visits, and visits to other community locations
What We Need to See:
- Bachelor’s degree with 1-year experience working with the homeless population.
- 1+ year's experience in one of the following: crisis intervention, counseling, mental health, substance abuse, domestic violence, housing procurement, or job development.
- Valid driver’s license and must be insurable under the agency’s automobile insurance policy.
- Required to work a flexible schedule.
