Job Summary
A thriving community mental health clinic is seeking an outgoing, energetic case manager who specializes in working with the Forensic/Criminal Justice population to serve as the Lead CM for a new Forensic Day Reporting Program. Please be organized and passionate about helping people in the mental health, substance abuse and competency fields.
Responsibilities and Duties Include:
Your primary duty will involve engaging clients that have been deemed Incompetent to Proceed by the Court through a Forensic Day Reporting Program. Programing will include providing Outpatient Restoration Services (Education Groups, Individual Sessions and Case management).
Responsibilities will also include, but will not be limited to, the following:
- Reviewing referrals and determining program appropriateness.
- Attending and participating in multidisciplinary treatment team meetings at the jail.
- Participating in court hearings as an agency representative.
- Assisting clients based on their needs (this may include intake paperwork, applications for insurance, housing, treatment homework, direct client support and other services).
- Referring clients to appropriate adjunct services.
- Navigating community resources.
- Completing, organizng, and submitting monthly documentation to the Office of Behavioral Health.
Qualifications:
Baccalaureate degree (or higher) in sociology/criminal justice, social work, psychology, or closely related field.
Experience in case management and/or community systems navigation with individuals with co-occurring disorders (preferred but not required).
Comfortable with the mentally ill and criminal justice population.
Requires exceptional flexibility, time management, and documentation skills.
Must be a self-starter and self-motivator.
Must possess a valid Colorado driver's license and have access to reliable transportation during work hours.
Desire to make a difference in the lives of people that truly need a reliable advocate.
Job Type: Full-time
Pay: $23.00 - $24.00 per hour

Due to our agency being a Medical Program, staff are required to obtain the COVID and Flu Vaccines .

- Pueblo, CO 81005: Reliably commute or planning to relocate before starting work (Required)
