PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Rapid Rehousing team as the Case Manager III at the RHC location.

ABOUT OUR TEAM
Our Rapid Rehousing and Prevention Programs provide services to assist individuals and families who are experiencing or at imminent risk of homelessness quickly obtain and sustain housing. Respectively, these programs provide temporary financial assistance and services to return people experiencing homelessness to permanent housing or temporary financial assistance to prevent people from falling into homelessness. These programs primarily serve participants a mid-lower level of acuity and work to rehouse clients in sustainable housing within a specific timeframe by providing move-in and/or temporary rent assistance.

WHAT IS A CASE MANAGER III
As part of the rapid rehousing team, the case manager provides field-based direct client services alongside a team of other case managers, housing
locators, employment specialists and community providers to clients participating in the program.

Position Responsibilities include:

  • Provide field based/mobile case management services in the areas of, but not limited to independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, linkage to community providers for substance abuse, primary and mental health care, and all other services needed to assist clients in reaching their goals.
  • Provide strengths-based case management and service coordination services designed to assist clients in obtaining and maintaining stable employment and housing.
  • Develop individualized treatment plan in collaboration with client addressing short-term and long-term goals.
  • Provide on-going case management support to assess progress and ensure treatment plan outcomes are met or changed as needed.
  • Provide services focused on enhancing the clients’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate own care.
  • Utilize evidence-based practices in service delivery such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention, and Housing First practices.
  • Perform housing location and retention services by conducting neighborhood canvassing, landlord outreach, unit inspections and providing support to resolve tenant & landlord disputes.
  • Work closely with program employment specialists to assist clients in obtaining and maintaining employment documentation.
  • Maintain documentation standards as set forth by the program contract and PATH policies, including maintaining compliance with HIPAA policies and practices.
  • Complete progress notes on every face to face/ telephone contact with client or collateral contact by the 5 p.m. the next business day.
  • Complete intake, assessment, service plans in coordination with client and within the documentation standards set forth by the program contract and PATH.

  • Strong written and verbal communication skills.
  • Strong interpersonal skills including the ability to interact effectively with a diverse group and build successful collaborative relationships with program participants, staff, and community partners.
  • Ability to maintain and execute confidential information according to HIPAA and all other applicable policies and standards.
  • Ability to work effectively both independently and as part of a team.

PREFERRED QUALIFICATIONS

  • Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.
  • HMIS Experience.

MINIMUM QUALIFICATIONS

  • All levels of education and experience welcome.
  • Demonstrated knowledge of and/or experience with advanced and evidence-based interventions including but not limited to:Critical Time Intervention

Harm Reduction
Crisis Intervention
Motivational Interviewing

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:Tuberculosis Test

  • Have reliable transportation

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
