Job description
POSITION SUMMARY
The Certified Addiction Counselor (CAC) functions as part of clinical team responsible for delivering rehabilitation services to a group of consumers receiving services from the Community Connections ACT Program under the supervision of the ACT Program director.
KEY RESPONSIBILITIES
- Facilitates rehabilitation, psycho-educational, and support groups.
- Serves as the chosen Rehabilitation Clinician as chosen by the consumer.
- Participates in the development of the ISSP and conduct ongoing periodic reviews, meeting with the consumer, family and/or other network members.
- Completes all documentation necessary for good clinical care and accurate billing. One hundred percent compliance is expected.
- Provides one-to-one counseling as requested by the consumer.
- Participates in training relevant to position and to the maintenance of license.
- Assists with the preparation of the program’s daily lunch.
- Liaison with CSA Clinical Manager.

- Protects the privacy of our consumer’s protected health information by maintaining compliance with HIPAA and other relevant CC related IT security regulations.
- Completes and stays current on role specific and organizational wide training.
- Performs other duties as assigned on an as-needed basis.
DESIRED KNOWLEDGE/SKILLS/ABILITIES:
- Associates degree in health or human services field required, Bachelor’s degree preferred
