Responsible for the initial screening and coordination of new participants to the San Diego program. Also responsible for working with the Marketing Manager to implement the program’s intake, outreach, enrollment, and marketing activities

Essential Functions:

Coordinates with potential participants and/or their families to conduct initial screenings and assessments in the homes of the potential participants.

Conducts San Diego program eligibility screening, intake, education and enrollment assistance activities to initially determine participant needs and eligibility for enrollment.

Completes appropriate documentation related to participant assessments as required.

Coordinates communications of eligibility decisions, approvals or disapprovals with participant or their family members.

Participates in the interdisciplinary team (IDT), management team and other related meetings and activities as needed.

Participates in outreach and marketing activities in conjunction with the Marketing Manager.

Collects and data enters the daily referral, intake, enrollment, marketing and outreach activities notes with submission of weekly and monthly reports to Marketing Manager.

Provide tours of center(s) to interested family members, potential participants, community partners and providers.

Provides responsive, high quality service to patients, employees, representative of County and outside agencies and members of the public by providing accurate, complete and up-to-date information in a courteous, efficient and timely manner.

Facilitates referrals and community resources for assistance with other needs and complex issues.

Provides input and feedback on internal and external outreach, education and enrollment plan for engaging, assisting, enrolling and retaining new and current patients.

Conducts public outreach and education activities, individually and in group, to patients and community members at clinic locations and other community locations about the program and other related health coverage options.

Multi-task in projects and their respective activities, timelines and issues.

Work independently and as effective member of the team.

Able to adjust outreach and enrollment strategies as necessary based on data analysis.

Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, providers and the general public.

Complies with all department, organization and government policies & procedures.

Attends meetings and trainings as required.

Education Preferred: Bachelor’s Degree in a health or human service field.

Education required: High school diploma or GED equivalent.

Experience Preferred: A minimum of one year working in community health, senior health, marketing, or enrollment for a public health, social service organization or health plan. One year of MediCal, Medicare, managed health care eligibility or senior related sales. Working with the frail or elderly population is a plus.

Certifications/Licenses: CA driver’s license with appropriate automobile insurance required. Must take and pass the State of California DHCS PACE and Medi-Cal Marketing Certification exam within 60 days of hire.

Verbal and Written Skills to Perform the Job: Must be able to communicate effectively and comfortably in public forums. Prepare concise and clear reports on marketing and outreach. Bilingual (English/Spanish) is preferred.

Technical Knowledge and Skills to Perform the Job: Good knowledge of Microsoft Word applications including Excel, Access and Power Point. Computer data entry, analysis and reporting experience is required. Knowledge of electronic health data systems including NextGen is preferred.

Competencies: Demonstrated ability to be culturally sensitive and respect diversity, work effectively with individuals of different cultures and socio-economic status; passion for service; self-starter and highly organized; ability to prioritize, and complete a large volume of work within strict time deadlines; provide prompt, efficient and responsive customer service.

Equipment Used: Computers, phones, personal vehicle, copy machines, fax machines, and other general office equipment.

Working Conditions and Physical Requirements: Busy Adult Day Health Center (ADHC) and clinic environment serving the frail elderly with medium to loud noise levels and controlled office temperatures. May be required to lift up to 30 lbs., stand and walk frequently indoors and outdoors; set-up/take down outreach equipment. Will be required to travel to other sites or other locations within San Diego county. Will be required to work some evenings and/or weekends.

Pay: $18.00 - $27.00 per hour
