Our Agency
Since 1990 the Front St. family of companies has been a leader in mental health support services in Santa Cruz and Monterey County. We are proud to be an integral part of a system that provides housing and mental health services for over 300 consumers. This position will be a member of our Community Services Team. Community Services specializes in providing case management, in-home support, housing navigation and wellness coaching for clients who live independently in the community. This is an Hourly Full-time position eligible for health benefits, 401K with match, holiday, and Paid Time Off (PTO).

The ideal candidate will be a recipient of Santa Cruz County housing support/navigation services but those with related lived experience are welcome to apply.
Responsibilities

• Assist Case Manager to provide street, shelter, and encampment outreach and engagement to enroll and serve participants in the Healing the Streets program.

• Provide knowledge, experience, emotional, social, or practical help to participants to support them in meeting goals.

• Attend huddles and meetings as part of their regular activities.

• Coordinate with the Street Medicine team to ensure that any medical or behavioral health needs identified by that team are addressed.

• Services will be documented in the Together We Care (TWC) platform and referrals will be made using the Unite Us platform wherever possible.

• Assist Case Manager to reach treatment and support goal for participants.

• Work with Case Manager to meet expected program outcomes.

Qualifications

• Peer Support Coaches will be trained in Critical Time Intervention (CTI) and meet any recertification requirements.

• Must be able to pass a Department of Justice criminal record clearance.

• Lived experience being unhoused is greatly preferred.

• A good driving record is required.
