GENERAL SUMMARY AND PURPOSE:

Provides management for development, planning, implementation, and evaluation of the SAHS Regional Heart Failure Program and ensures the systematic and coordinated delivery of clinical services that include pre and post heart transplant care, pre and post durable LVAD care, advanced heart failure, pulmonary hypertension and cardiogenic shock across multiple clinic and hospital locations.

Responsible for the development, communication, and successful execution of established objectives as articulated through the organizational structure and the strategic and tactical plans. In addition to management responsibility and clinical consultation of staff while supporting an integrative medicine approach to patient care, research and community education and leads outreach efforts within the region. Responsible for the overall performance of the assigned program including collection and measurement of applicable Trinity and national indicators for quality patient care.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

Must have a Bachelor of Science in Nursing. Colleague must have an RN license, as defined by their primary work state (Idaho or Oregon), issued prior to hire date. Licensed as a Registered Nurse in the state of Idaho. Must receive Registered Nurse license in Oregon within six months of hire.

Three years of leadership experience in a hospital or clinic setting. Three years of experience in developing and coordinating clinical programs, inpatient and outpatient experience preferred. Education in, and/or experience with, chronic diseases and disease self-management, masters prepared highly preferred.

ESSENTIAL FUNCTIONS:

Knows, understands, incorporates, and demonstrates the Organizational Mission, Core Values, and Vision in behaviors, practices, policies and decisions. Maintains a competent level of care RN’s as demonstrated by the nursing process involving assessment (collects comprehensive client health data), diagnosis (analysis of assessment data), outcomes identification, planning, implementation and evaluation. Successfully executes the established plans and the services of the functional areas of responsibility through analysis, measurement and monitoring, reporting, and adjusting the direction, allocation and utilization of available resources.

Facilitates provision of quality cost-effective patient care in alignment with established goals and priorities based on physician, patient/family needs, staff needs and program objectives. Develops and manages implementation of an annual program evaluation plan and performance improvement activities, including quality improvement projects.

Revenue management: Responsible to work with the Director to develop budgets and forecasts for new programs and understands charge capture, revenue reconciliation, capital acquisitions, pro forma, coding and billing in order to achieve programmatic growth objectives.

Assists in preparation of annual forecasts, monitors performance and adjusts expenditures. Develops policies and procedures to ensure compliance with hospital, state, federal, and accrediting agency guidelines. Keeps informed about agency changes in assigned program and related policy, procedures, and equipment. Provides comprehensive clinical coordination of care including an interface with case management functions.

Assists Director in recruiting, hiring, orienting, developing, and evaluating performance of applicable staff. Ensures employees' orientation and performance goal setting to promote professional development. Develop an educational model across multiple specialties (palliative care, social work, ED, inpatient and outpatient RNs) to ensure competency in LVAD, cardiogenic shock and pulmonary hypertension care that meets national standards Ensures completion and keeps up to date per regulatory requirements staff competency assessments Mentors other members of the care team.

Promotes a culture of collegiality that enables members of multidisciplinary team to feel respected and valued. Utilizes effective leadership and management skills to inform and engage staff and maximize their potential. Ensures systems to improve staff competence. Assists staff in developing their skills and defining educational needs. Monitors and maintains appropriate licensures and certification documentation for staff. Ensures staff performance is consistent with a physician, staff and patient/family friendly environment.

Recommends new ideas or services to help grow business and meet patient, provider, and other community needs. Recommends changes to space, workflow, staffing, and scheduling to improve services or increase volume. Develops and maintains relationships and partnerships with community groups, payers and potential referral sources.

With the Advanced Heart Failure Medical Director, ensures that all applicable program accreditation requirements are met and maintained. Handles patient and staff complaints; adjusts systems to minimize concerns and respond timely. Establishes and maintains positive and productive working relationships with all relevant customers and suppliers.

Assures compliance with all applicable system policies and procedures.

Demonstrates competence in areas of direct patient care, clinical staff and patient education, collaboration, research participation and utilization, and administrative activities. Facilitates plan of care and symptom management with outpatient staff. Supervises and supports Heart failure, transplant and VAD coordinator roles to help triage and facilitate timely care Coordinates information flow among medical, ancillary, nursing, financial and insurance offices.

Collaborates with physicians to plans and evaluates the delivery of care. Manages the health care of patients with VADs including responses to any patient safety events. Assists in obtaining medications and supplies from the appropriate source according to the patient's insurance coverage. Ensures equipment, supplies, and charges in conjunction with hospital liaisons are managed and appropriate.

Assists in reinforcement of discharge information, and trains community for patient discharge. Contacts referring physician and arranges follow up in the local area or at CCF depending upon patient and their activity. Supports requirements for continued VAD Certification by Joint Commission. Participates in research by the implementation of protocols and the collection of data.

Facilitates procedures which enable the implementation of studies with various interdepartmental components. Provides in-service for nursing staff and presentations for any outside organization or group inquiring about the transplant or VAD process. Supports the VAD Process Improvement Committee preparations as required.
