Position Overview

Blaze Health is seeking a dynamic, experienced, Hospice Nurse Practitioner to join our Hospice care team visiting customers in their homes, assisted living and other health care facilities. The Certified Nurse Practitioner, in collaboration and consultation with the physician(s), staff RNs and other health care professionals, will work with the customer and her or his family to identify, control or alleviate physical and emotional discomfort and other symptoms that accompany a serious health condition. Practices within the context of collaboration with the physician(s) in diagnosing and managing symptoms related to terminal illness and end of life care. Nurse Practitioners have an advanced nursing practice that includes independent nursing functions based on nursing standards of care and a role in medical management working within a collaborative agreement with the physician(s). The Nurse Practitioner provides leadership by serving as clinical expert, educator and liaison between the customer and the medical, nursing and interdisciplinary staff with the goal of providing remarkable care.

Master’s or higher degree in Nursing required.

Certification prior to the Masters’ degree requirement and current practice as a Nurse Practitioner in their area of specialization or documentation of current competency.

Hospice Experience preferred

Two or more years’ experience in a Nurse Practitioner role preferred.

One or more years’ experience in area of specialization preferred.

Licensure/Certification(s) Mandatory

Current licensure as RN and APRN in State of Minnesota

Valid DEA Registration.

Current medical staff credentials for level of training.
