Looking for a Direct Support Worker to provide care for a special needs, wheelchair bound, 20-year-old female client. She will need assistance with ADLs/IADLs, bathing, toileting (which includes urethral and appendicovesicostomy access), medication administration (via oral and mic-key button gastrostomy), meal prep and assistance with home exercise plan as prescribed by OT/PT. Companion care and chaperoning for community outings like to the mall, nail salon and library are part of her cares, as well. Some of her cares are delegated by an RN; Both of her parents are medical personnel and will provide training and ongoing support, to the caregiver. Client’s mother is an RN who works from home and will be home during these shifts and can handle the delegated duties such as meds and catheter if the DSP is not comfortable with that. But if they would like, they can learn them.

Schedule: The family is looking for a DSP to work two full days a week and occasional weekend nights for about 20 hours per week. They are flexible on the exact days but would strongly prefer at least one of them to be a weekend.
Employees looking for more hours can work with additional clients at different times.

Direct Support Professional (DSP) / In Home Care Giver Responsibilities and Duties:

This role involves caring for a client on an individual basis in their home environment and taking them out into the community to learn skills and socialize. This role varies depending on the client but can involve teaching skills of independent living such as self-care, behavior management, budgeting, cooking, shopping and cleaning. Family Relief Resources is not a physical facility -100% of activities happen in the community or a client’s home.

Staff can choose how many clients to work with and can collaborate with the families to create a weekly schedule. We need enthusiastic, patient, caring, creative and dependable support workers that are ready to make a difference and integrate into their client’s family with love and kindness.

The day-to-day duties of the job require:

* Smart Phone. The state requires time be kept on their website with GPS location verification. You will need a smart phone to clock in and out for each shift at the client’s location.

* Driving. Have a valid license and car with current insurance. Not all clients will need transportation, but most do.

* Documenting. Must keep specific progress notes weekly.

* Scheduling. Each DSW has a set schedule and must be able to maintain this with clients.

* Reliability. Because Family Relief Resources is not a facility, we need reliable employees who can work independently, show up for their shifts, and be an excellent communicator with their client’s families and with our agency.

* Patient, Kind, and Positive. Helping those with disabilities requires a person who can be patient in all situations, show kindness, and have a positive attitude toward the client and other members of the client’s family even in challenging situations.

* Flexibility. Being able to adjust to different types of personalities and disabilities from day to day.

Direct Support Professional / In Home Care Giver Required Experience:

  • 1 year preferred but not necessary.

Job Requirements:

  • 18 years or older
  • Pass a Criminal History Check.
  • Valid driver’s license and insurance.
  • CPR and First Aid Certificate (within3 months of hire)
  • State required online training courses (pre-hire and within 3 months of hire)
