Responsibilities and Duties

Come join our Team! Rocky Mountain Care has an immediate opening for an Intake Coordinator. This position coordinates admissions for Home Health & Hospice, working with our nursing team to ensure the best possible care for our patients!

  • Obtains pertinent pre-admission documentation from referral sources to determine if agency can care for the patient adequately
  • Review clinical documentation to determine level of care needed
  • Develops and maintains relationships internally and with community partners
  • Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families

Qualifications

  • High school graduate or equivalent
  • Demonstrate excellent customer service both in-person and over the phone.
  • Ability to work independently and as a part of a team.
  • Basic computer knowledge
  • Demonstrate excellent organizational and analytical skills.
  • Ability to multitask in a fast-paced environment
  • Good verbal and written communication skills
  • Experience in health care preferred but not required

Schedule

  • Monday-Friday (rotating shifts during the hours of 8:30am-6:30pm)
  • One scheduled weekend every 6 weeks

Job Type: Full-time

Pay: From $17.00 per hour
