Serenity Hospice Care is looking for an organized and compassionate Intake Coordinator to join our team! The Intake Coordinator will be responsible for working with hospice patients and their families, they discuss services and act as a liaison between facilities, marketing, social workers and nurses. The Intake Coordinator will report to the Director of Admissions and will relay all information as needed.

Responsibilities:

• Offer professional and positive interactions with clients and their families.
• Process and document new referrals.
• Track census daily for assigned territory.
• Track and document level of care for patients.
• Work with staff to place patients in appropriate facilities.
• Keep Director of Admissions informed of admissions taking place.
• Contact referral sources as needed to receive updates on intakes.
• Track documentation for compliance purposes.
• Track documentation as needed for billing.
• Provide weekly feedback status to the Admissions team.
• Answer phones and filing as assigned.
• Other duties as assigned.

Requirements:

• Previous experience medical office experience a PLUS!
• Excellent communication skills - verbal and written.
• Proficient skills in Microsoft Office.
• Reliable transportation and valid drivers license.
• Ability to multi-task and work under pressure.
• Organized and self motivated.

Serenity Hospice Care is looking for an experienced In Take Coordinator for one of our NJ Territories.

The qualified candidate must be able to speak with hospice patients and their families to explain our Hospice Services, speak to staff in multiple facilities, coordinate admissions with Serenity Marketers, Social Workers and Nurses.

This person must be extremely organized and detail oriented.

Must have exceptional phone etiquette and follow up skills

Insurance Experience preferred

Must be computer literate. Extremely proficient with Word, Excel, Power Point.

Experience with Homecare Homebase a plus

Must have at least two years in take experience in a high volume, fast paced atmosphere

Full Time Position Monday through Friday.
