Summary:
The clinician will have clinical supervision of the IOP LCSW/LMSW's in the Outpatient Department of Psychiatry in Norwalk. Will provide support as needed, review clinical documentation, and help manager develop and grow new programs. Review documentation for the other social workers to maintain compliance as well as provide education on clinical documentation.

Responsibilities:
1.

Provides support to LCSW/LMSW’s with regards to all Intensive Outpatient Programs.

2.

Facilitates/co facilitates group therapy, provides individual, family, crisis intervention, patient-centered support and psycho-education.

3.

Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral. Identifies potential barriers, limitations & lack of coping abilities, adjustments, and functional status. Participates in patient care planning.

4.

Conducts intakes with patients/family members to evaluate coping abilities, dysfunctional adjustments, maladaptive behaviors, functional status.

5. Maintains knowledge of community resources, refers patients and families to agencies for supportive services with concrete resources (financial assistance, housing, transportation; medications) on regular & emergency basis.

6. Documents charting requirement to meet Quality Improvement and compliance objectives by the department and external agencies and to assure optimal clinical and administrative record keeping.

7. Collaborates with the department and third-party payers to assure payment for clinical services rendered.

8. Development and updates of curriculum for all Intensive Outpatient Programs with Department Manager.

9. Achieves the organization established expectation with regard to quality service, collaboration, and teamwork.

10. Completes random chart reviews to ensure all documentation is completed and in accordance with compliance.

11. Evaluates for child, elder abuse, and domestic violence and makes appropriate referrals. Acts as a consultant and/or educator to staff relative to assessment, criteria & referral.

12. Ensures weekly treatment meeting for continuity of Participates in weekly treatment team meetings for all IOP.

13. Establishes and maintains relationships with community providers, schools, etc as referral source.

14. Performs other duties as assigned.
