Foundation Home Health is in search of motivated LPNs interested in making a real difference in the lives of their patients and joining our team as we set a new standard for home health care in Lake Havasu City, AZ. We are currently hiring for ALL nursing positions to expand our patient care team.

We are a Medicare Certified Agency and Accredited by ACHC.

We are looking to fill positions in Bullhead City at this time.

General Requirements for all positions:

• Currently Licensure/Certification required for the position
• Arizona Level One Fingerprint Clearance Card (for more information https://www.azdps.gov/services/public/fingerprint )
• Current Healthcare Provider CPR Card
• Current TB test results
• Current auto insurance AND reliable transportation
• One or more years in Home Health or Hospice preferred

Job Types: Part-time, PRN, Per diem

Pay: $42,000.00 - $60,000.00 per year
